It is a patient’s right to…
- Receive qualitatively reliable and safe medical services;
- Have their personality and their individual views on healthcare respected;
- Express their opinions and wishes, refuse medical services that they do not wish to receive, and seek a “second opinion” from another physician at another medical institution;
- Receive adequate explanation and information on healthcare in an accessible language and manner until they attain a satisfactory understanding;
- Make their own decisions regarding medical treatment and other such matters, based on the adequate explanation and information they have received on the subject matter;
- Have the hospital protect their personal information that it has obtained in the process of treating them;
- Receive adequate information on the purposes, risks, and other relevant matters of clinical studies and investigational medical treatment proposed to them, determine on their own whether or not to accept such proposals, and discontinue their participation in investigatory treatment at any time, even before the end of the term.

- Provide us with as accurate information as possible on your health to ensure appropriate medical care;
- Ask questions if you do not understand anything when you are given information about the medical treatment that is proposed to you or that you are receiving;
- Adhere to the rules and instructions that are given to you in connection with your treatment, and let us know if you feel unsure, anxious, or worried about anything in the course of your treatment;
- Contribute to realizing an optimal environment at the hospital so that all patients can benefit from medical services in the best possible manner;
- Accept the presence of, and cooperate with, residents (trainee doctors) and medical/nursing students at the hospital (designated as an institution for healthcare education and training) for their training, clinical practice, and educational visits.

General Consent for Providing Medical Care
The Mitsui Memorial Hospital’s general policy requires that we explain the procedures and tests necessary for examinations on written forms and obtain consent from patients before actually performing them.
The following tests and procedures, however, are exempt of such policy and will be performed as a part of care without additional explanation or obtaining consent because they do not require physician’s attendance and pose little stress to the patients’ mental/physical health (“general consent”).
Please do not hesitate to ask physicians/nurses if you have any questions. Please note that this general consent also covers local anesthesia. Please be sure to inform a physician /nurse if you have any allergies.

General items
Physician ’s interviews, visual examinations (including palpation and visual inspection), physical examinations, measuring of body temperature, body weight, and/or blood pressure, rehabilitation, instructions by dieticians, and preparation of meal menu, photography of wounds, affected areas, etc., and video recording of tests, procedures, treatments, etc.
Tests
Blood test, urine test, microorganism test on feces, urine, and/or sputum, pathological examination or cytodiagnosis of samples, electrocardiogramandelectroencephalograph, physiological tests including pulmonary function test, ultrasound examination, respiration test, expiratory gas analysis, and electromyography, general or fluoroscopic photographing of X-ray imaging, CT/MRI/RI examination without contrast media, psychological test, monitoring ofpercutaneousspO2, arterial pressure, breathing capacity, and muscle relaxation, skin reaction tests (e.g., patch test, intradermal reaction test, scratch test, tuberculin reaction, minimal erythema dose), and skin test for allergies, pharyngeal fiberscopy.
Monitoring
Please note that we may be monitoring the operation rooms, ICU, CCU, radiology treatment section, MRI/CT rooms, physiology labs, and neonatal care section, as needed, using live cameras.
Procedures
Suction of phlegm, etc.;transnasal gastric tubing;bladder catheterization; tests and treatment for dental caries, periodontal disease, or dentures; oral care.
Administration of medication
Ordinary administration of medication including injections, securing an intravenous line, securing a sustained subcutaneous indwelling needle, oxygen treatment
Anesthesia
Local anesthesia involved in tooth extraction or sutures. (Please be sure to inform the staff if you have any allergy.)
The procedures listed above are performed by a skilled professional with a certain level of experience, but the patient may experience complication including bleeding. Treatment for such complication will be provided as a part of the normal medical care as provided under the national health insurance for those who are eligible.
Please note that any test or procedure may involve some degree of pain. We will provide a pain reliever of some kind when the pain is intense. We may provide a preventative measure for likely pain.
Medical procedures other than the general consent items listed above will be performed after obtaining prior consent from the patient. However, in the event of a life-threatening emergency, medical procedures may be performed without obtaining various types of prior consent forms.
In addition, when assistance is required due to language, vision, hearing, or other reasons, oral consent may be obtained via interpreter, braille communication, sign language, or other appropriate methods. We ask for your understanding in advance.
* In the event of any discrepancy in interpretation or translation between Japanese and a foreign language, the Japanese version shall take precedence. In addition, all medical care provided at this hospital shall be governed by and construed in accordance with the laws of Japan. In the event of litigation, the Tokyo District Court shall have exclusive jurisdiction as the court of first instance.
